We have an exciting opportunity for an experienced nurse to join the Neuro Rehabilitation Unit at Neath Port Talbot Hospital. The post holder will provide specialist nursing care to people with neurological conditions who are referred to the Neuro Rehabilitation Unit. The applicant will provide comprehensive specialist, multifactorial assessment, plan, implement, and evaluate evidence-based nursing care. This role involves autonomous, independent clinical practice and close collaboration with Neurology consultants and clinical nursing staff within the department. Key responsibilities include: Providing specialist education and training to staff. Caring for a varied case load of neuro-rehabilitation patients with multi-pathologies and severe physical and cognitive impairment. Working collaboratively with multidisciplinary clinical colleagues to ensure comprehensive care to the patients. Undertaking audit, supporting clinicians with their clinics, and performing ward-based work when needed. Conducting telephone consultations. Developing the nursing role in the Neuro-rehabilitation service, including mentoring junior nurses or AHPs and rehabilitation assistants. Screening referrals, presenting at multidisciplinary team meetings, arranging admissions and discharges, and prioritizing patients on the waiting list. Assessing, differentially diagnosing, rehabilitating, and managing a case load of adults with neurological conditions. Conducting highly specialist assessments of neurological conditions, carrying out diagnostic procedures, and interpreting results for neurology patients with complex presentations. Providing comprehensive education and support to patients and healthcare providers. Participating in ward rounds, outpatient clinics, and multidisciplinary meetings. Providing specialist clinical knowledge to clinical colleagues, patients, and carers/relatives. Supporting a culture that promotes person-centered, safe, and effective care. Establishing and maintaining effective communication with patients, carers/relatives, and professionals across health and social services. Seeking appropriate advice/alerting agencies regarding any concerns about potential/actual risk to patients or their dependents (e.g., Child Safeguarding, domestic violence, protection of vulnerable adults, mental capacity, and deprivation of liberty).
