Location: Godalming
Duration: 3 - 6 months ongoing
Schedule: Mon - Fri, 9 - 5
Start Date: June 2025
Rate of Pay: £25 - £27 per hour
What You’ll Be Doing:
- Join our team as a CPN and play a key role in improving the lives of working age adults.
- Perform comprehensive patient assessments.
- Create and implement personalised care plans.
- Deliver evidence-based treatments to optimise patient outcomes.
- Collaborate with multidisciplinary teams to provide holistic care.
- Maintain accurate patient records.
- Visit patients out in the community.
Requirements & Qualifications:
- A current and valid NMC registration and pin.
- Experience working with older age adults in the community.
- Strong communication and interpersonal skills.
- The ability to work independently and within a multidisciplinary team.
- A commitment to delivering high-quality patient care.
- Access to a car for community/home visits.
Working Hours & Contract Duration:
This role offers a flexible working position requiring availability from Monday - Friday with flexibility where possible. The contract is for 3 to 6 months with a strong possibility of extension based on performance and service needs.
