We are seeking motivated and dynamic Band 6 Community Nurse to join the Specialist Children’s Community Nursing Team in providing excellent quality nursing care for children with Complex Needs attending Special School in Waltham Forest. The Specialist Children’s Community Nursing team comprises four nursing teams – Child Development Nursing, Special School Nursing, Paediatric Continence, and Children’s Community Nursing team. The aim is to provide an excellent standard of nursing care to enable children to achieve their best possible outcomes.

The post holder will utilise a range of skills and experience gained in clinical practice to provide holistic assessments for children who have complex needs. Clinical judgement will aid the post holder to formulate a care plan based on these assessments. The post holder will also demonstrate excellent time management skills to enable them to work autonomously at times. The role will require the successful candidate to work as part of the team based at Wood Street Health Centre and at Specialist Educational Premises, but will also have the opportunity to undertake home visits as part of the role.

Main duties of the job

  • To plan, implement and evaluate nursing interventions within community settings, including Specialist Education Provision for children and young people.
  • To support the transition processes for children and young people with complex Health and Special Needs, taking the lead where appropriate.
  • To work with Social, Educational, Health Colleagues, parents and young people to develop joint care plans and packages of care.
  • To involve parents, children and young people and encourage them to make informed choices concerning their health and well-being.
  • To work without direct supervision at times, and to be responsible for the management of own workload.
  • To provide up to date and skilled nursing care for example: care of enteral feeding devices, management of seizures, care of tracheostomies, suctioning and other interventions.
  • To participate in the identification, referral and support of children and young people with additional needs.
  • To compile health care plans for children and young people in consultation with the wider multi-disciplinary team, child, and family.
  • To contribute to individual Education, Health, and Social Care plans.
  • To maintain and to further develop own professional practice in line with the needs of the service.

Person specification

Qualifications

  • Holds current NMC UK registration
  • Registered Children's Nurse
  • Degree in School Nursing / Public Health / Community Health Course
  • Qualification in teaching/learning

Experience

  • 2 years minimum post-registration experience of acute paediatrics
  • Evidence of multidisciplinary working and liaison related to children and young people
  • Working within the community setting, within a school nursing team or children's setting
  • Minimum of 1 year experience of working with children and young people who have complex health care needs

Skills

  • Basic awareness of IT and IT skills
  • Demonstrates excellent written and verbal communication skills

Knowledge

  • Aware of own abilities and boundaries and to be able to carry out the clinical role without continuous direct supervision
  • Demonstrates knowledge of care planning process
