As an Assertive Outreach Support Worker, you will be the driving force behind our mission to empower and support individuals in need. This temporary role, based in Yeovil, offers the opportunity to make a tangible difference in the lives of the community.

Preferred Requirements:
- Proactively engage with individuals in known hotspots, building trust and rapport to understand their unique needs and challenges.
- Coordinate and deliver tailored support services, ensuring seamless access to resources and opportunities.
- Advocate on behalf of clients, navigating complex systems and advocating for their rights and wellbeing.
- Collaborate with a multidisciplinary team, sharing insights and coordinating efforts to achieve the best possible outcomes.
- Maintain detailed records and documentation, contributing to the continuous improvement of our service delivery.
