Salary: $78,480 / year Sign-on Bonus: $5000; $1666 upon 30 days, $1667 upon 6 months, $1667 upon 12 months. The Lead Clinician for Program for Assertive Community Treatment (PACT) works as part of a multi-disciplinary team providing treatment, rehabilitation, and support services to adults with severe and persistent mental illnesses. Under the supervision of the Team Leader and Psychiatrist, the Lead Clinician provides clinical leadership in treatment planning, coordinates Individual Treatment Team (ITT) duties, and ensures complete and accurate documentation. Provide case management and ongoing assessment for assigned persons served, including coordinating and monitoring ITT activities. Develop, write, implement, evaluate, and revise the Individual Action Plan (IAP) goals in conjunction with ITT. Provide individual supportive therapy, symptom management, and educate/support families, advocating for persons served rights and preferences. Conduct comprehensive assessments covering psychiatric history, mental status, diagnosis, health, substance use, education, employment, social development, daily activities, and family structure. Act as liaison and consult with community agencies and families to coordinate treatment. Provide on-call crisis intervention. Perform other duties as required. Why Vinfen?
