We are recruiting for a well-known provider within the West Midlands. As an Advanced Nurse Practitioner (ANP), you will play a pivotal role in delivering high-quality, patient-centred care within our multidisciplinary team. Operating autonomously and collaboratively, you will assess, diagnose, treat, and manage patients presenting with a wide range of acute and chronic conditions. Your expertise will contribute to the development and implementation of evidence-based clinical guidelines, ensuring optimal patient outcomes and service delivery.
Key Responsibilities:- Clinical Assessment & Diagnosis: Conduct comprehensive assessments, including history-taking, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans.
- Patient Management: Prescribe medications (as an independent prescriber), initiate and manage treatment regimens, and provide follow-up care.
- Chronic Disease Management: Lead clinics for long-term conditions such as diabetes, hypertension, and asthma, implementing personalized care plans.
- Clinical Leadership: Provide mentorship and support to junior nursing staff, fostering a culture of continuous professional development.
- Quality Improvement: Participate in clinical audits, research initiatives, and service evaluations to enhance care quality and patient safety.
- Collaboration: Work closely with GPs, consultants, and other healthcare professionals to deliver integrated care pathways.
- Health Promotion: Develop and deliver health education and promotion programs to patients and the wider community.
- Record Keeping: Maintain accurate and up-to-date patient records in accordance with legal and professional standards.
- Registered Nurse with a valid NMC PIN.
- Masters degree in Advanced Clinical Practice (or working towards).
- Independent Prescriber status (V300).
- Minimum of 2 years' post-registration experience in a relevant clinical setting.
- Demonstrated competence in advanced clinical assessment and decision-making.
