The Home First Neighbourhood Team is seeking an Occupational Therapist with experience in hospital discharge to join their team.

What will your responsibilities be?

  • Conduct Care Act Assessments
  • Perform Mental Capacity Assessments
  • Make Best Interest Decisions
  • Manage hospital discharge processes to ensure smooth transitions for patients

Requirements:

  • Professional registration with Social Work England
  • Experience with hospital discharge
  • Ability to regularly attend John Radcliffe Hospital

Qualifications & Experience:

  • A degree or diploma in Occupational Therapy
  • Registration with the Health Care Professionals Council (HCPC)
  • A full UK driving license
  • The legal right to work in the UK
