The Home First Neighbourhood Team is seeking an Occupational Therapist with experience in hospital discharge to join their team.
What will your responsibilities be?
- Conduct Care Act Assessments
- Perform Mental Capacity Assessments
- Make Best Interest Decisions
- Manage hospital discharge processes to ensure smooth transitions for patients
Requirements:
- Professional registration with Social Work England
- Experience with hospital discharge
- Ability to regularly attend John Radcliffe Hospital
Qualifications & Experience:
- A degree or diploma in Occupational Therapy
- Registration with the Health Care Professionals Council (HCPC)
- A full UK driving license
- The legal right to work in the UK
