Yowsun Care is a dynamic, vibrant and growing Care Agency that prides itself on providing excellent homecare services to its clients. We are looking to recruit an experienced Field Care Coordinator to join our Wiltshire Branch. You must have a UK driver's licence but not necessarily own a car of your own. THIS POSITION IS NOT AVAILABLE TO ANYONE REQUIRING A SPONSORSHIP TO WORK IN THE UK.
The Field Care Coordinator will work closely with the Registered Manager and the Deputy Manager, undertaking new client assessments, conducting care plan reviews and risk assessments, writing care plans, planning rotas, briefing carers prior to care calls, conducting supervisions, spot checks and medication competencies and managing client visits, whilst also supporting staff members with access to their calls, when required.
Responsibilities:- To carry out care plan/risk assessments of all new clients within 48 hours of referrals.
- Start providing the initial services and then pass onto a carer when the package is settled.
- To write all care plans and risk assessments and update these accordingly.
- To support service users and care staff by coordinating their visits and calls.
- To work closely with the care needs assessor and other team members to enable the smooth running of the service.
- Liaise with the registered manager to ensure that there is enough cover to deliver all visits and that new clients are planned into the rota in a timely fashion.
- Ensure that care workers with the correct skills and experience are matched to deliver excellent care to our clients.
- Ensure working patterns are efficient and planned logically.
- Effectively prioritise tasks to meet unexpected emergencies and changes in care packages.
- Appropriately match care workers to clients, considering care worker skills, experience, travel arrangements and client requirements and preferences.
- Complete and distribute rotas on a weekly basis.
- Arrange cover for care worker sickness, absenteeism, or holidays.
- To carry out introductory visits every time a Care Worker visits a client for the first time.
- To ensure all client visits are always covered.
- To be on-call in case of emergency.
- To monitor, supervise and mentor all Care Workers in the field.
- To support staff members with access to their calls, when required.
- To provide support to all Care Workers in the field and carry out spot checks, medication competencies and supervisions.
- To monitor all customers for service and Care Worker satisfaction.
- To encourage and identify opportunities for Care Workers continuing professional development.
- To provide and facilitate in-field training as and when appropriate.
- To facilitate the practical induction of all new Care Workers.
- Experience in a care coordinating capacity within Domiciliary Care.
- Excellent organisational skills - ability to plan and organise workloads effectively to ensure that clients receive the services that they expect and deserve.
- Ability to maintain clear and accurate written records and follow statutory reporting procedures.
- Ability to communicate clearly and build positive working relationships with clients, their families, fellow staff and other health and social care professionals.
- Excellent telephone manner and interpersonal skills.
- Ability to work under pressure and to meet deadlines, in a calm and patient manner.
- Be computer literate, able to proficiently use Microsoft 365 packages and able to learn the in-house roster system, including the use of apps.
- NVQ Level 3 preferred.
- Driving is essential with the ownership of a valid driver's licence.
