What You'll Be Doing:
- Join our team as a Band 6 Social Worker and play a key role in supporting the older adults with mental health needs across three integrated teams in Bromley.
- Perform comprehensive patient assessments.
- Caseload holding in a specialist older adult mental health team.
- Create and implement personalised care plans.
- Deliver evidence-based interventions to optimise outcomes.
- Collaborate with multidisciplinary teams to provide holistic care.
- Maintain accurate patient records.
Requirements & Qualifications:
- A current and valid SWE registration.
- Experience working with adults or older adults in mental health services.
- Strong communication and care planning skills.
- The ability to work independently and within a multidisciplinary team.
- Confidence working in a fast-paced and integrated setting.
Working Hours & Contract Duration:
This flexible working role requires availability from Monday to Friday, with part-time options considered. The contract is for 3 months with a strong possibility of extension based on performance and service needs.
