For Companies We are looking for a highly motivated and experienced Senior Clinical Pharmacist to join our progressive healthcare team in Reading. This is a pivotal role for an individual who is passionate about improving patient outcomes, advancing clinical practice, and leading innovation in medicines management within primary care or integrated care systems.
As a Senior Clinical Pharmacist, you will provide strategic clinical leadership across prescribing, medication safety, and clinical governance.
Key Responsibilities:
- Lead on complex medication reviews and medicines optimisation for patients with polypharmacy, frailty, or chronic conditions
- Provide clinical leadership on prescribing initiatives and quality improvement projects
- Support and develop junior pharmacists and pharmacy technicians
- Lead the implementation of local and national prescribing guidelines and formularies
- Act as a key advisor on medicines-related queries from clinicians and patients
- Collaborate across multidisciplinary teams, including GPs, nurses, and allied health professionals
Requirements:
- GPhC-registered pharmacist with substantial post-registration experience
- Independent Prescriber qualification is essential
- Proven experience in a clinical pharmacy role, ideally in primary care, hospital, or PCN settings
