Service Care Solutions are currently recruiting for Urgent Community Response Practitioners to join a busy and supportive NHS team on a full-time agency basis. About the Role:
- Respond rapidly to patients in crisis within the community to avoid unnecessary hospital admissions.
- Work autonomously and within a multidisciplinary team to assess, treat, and support patients in their homes or care settings.

- Liaise with GPs, social care, community teams, and acute services to ensure continuity of care.
