Service Care Solutions are currently recruiting for Urgent Community Response Practitioners to join a busy and supportive NHS team on a full-time agency basis. This is a fantastic opportunity for experienced clinicians looking to make a real impact in urgent care and crisis intervention in the community.

About the Role:

  • Respond rapidly to patients in crisis within the community to avoid unnecessary hospital admissions.
  • Work autonomously and within a multidisciplinary team to assess, treat, and support patients in their homes or care settings.
  • Deliver high-quality, patient-centred care with a focus on recovery, safety, and independence.
  • Liaise with GPs, social care, community teams, and acute services to ensure continuity of care.

Who We’re Looking For:

  • Qualified Registered Nurse, Paramedic, or Allied Health Professional.
  • Experience working in urgent care, community health, crisis response, or A&E.
  • Able to work independently and manage caseloads under pressure.
  • Excellent clinical decision-making and communication skills.
