An exciting job role has recently become available at the Rotherham Metropolitan Borough Council. They are looking for a new member for their Transitions Team.
As a Social Worker in the Transitions Team you will support young people with disabilities as they transition from children’s to adult services. Your responsibilities will include:
- Conducting comprehensive assessments to identify needs and appropriate interventions
- Developing personalized transition plans
- Providing emotional support and counselling
- Coordinating resources and services to aid in the transition process
- Monitoring and evaluating the progress of clients
- Facilitating communication between clients and relevant parties
- Assisting with job placement or educational transitions
- Advocating for clients’ needs and rights
- Organising and conducting workshops or training sessions to support their development

To be successful in this role you will need strong communication skills, capable of effectively and empathetically engaging with diverse groups. The ideal candidate will possess empathy and resilience, understanding and sharing the feelings of others while maintaining resilience in challenging situations. They should have excellent problem-solving abilities, able to assess situations and develop appropriate support plans. Additionally, candidates must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree-level or equivalent qualification in Social Work are also required.
