Home Care RN Case Manager

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager.

Coverage needs primarily in and around the Douglas, Oxford, Sutton, Webster, Dudley, Uxbridge, MA area.

Monday-Friday, 40 hours/week
Competitive pay rate plus mileage reimbursement
No weekends, holidays or on-call required!
13-week contracts

The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN\’s and Home Health Aides.

Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
