Derby City South PCN is looking for a Community GP to help deliver the Acute Home Visiting Service - 'Team Up'. The post is for 37.5 hours, however we are happy to consider part time applicants, provided you are able to work on Mondays and Tuesdays. Your own transport will be essential to travelling across the PCN. This is a unique opportunity to join a new service delivering care to the most vulnerable and complex patients within the PCN. The postholder will work within a multidisciplinary team supporting patients to age well.
Home visits and proactive care in the patients own home, as well as integrating with and leading the broader community response, are key to this role.
Main duties of the job
- To work flexibly across a wide range of service timetables within a community care setting, providing face to face consultation sessions, clinical sessions, telephone triage, home visits, case management and clinical administration.
- To review and action diagnostic tests promptly to provide medical support/advice to other primary and community care colleagues.
- To promote and implement 'end of life' strategies, delivering high quality palliative care.
- To facilitate and support advance care planning including ceilings to care plans.
- To provide medical input and cover to virtual wards, care homes, community beds/residential nursing.
- To work closely with other primary and community care colleagues to deliver high quality patient centred clinical care. This will include where appropriate attendance/participation in multi-disciplinary team meetings (MDTs).
About us
Derby City South PCN is a Primary Care Network covering the south of Derby City, and the north of South Derbyshire district. Consisting of seven practices and a 100,000 population, this is a large PCN delivering the Derbyshire 'Team Up' specification - an ageing well program aimed at integrating care for housebound and complex patients. The PCN places patient care at the heart of everything we do, with safe, effective care that acknowledges and ensures dignity being of the utmost importance.
You will be employed by Derby City South Healthcare Limited - the delivery arm of the PCN.

Person Specification
- A vocationally trained and accredited GP.
- Current registration with GMC.
- On the GP Performers List.
- Full UK Driving Licence and own transport.
- MRCGP.
- Recognised qualification in care of the elderly.
- Recognised qualification in medical education/clinical supervision.
Experience
- Experience and evidence of an interest in care of the Elderly.
- Understanding of adult safeguarding and Deprivation of liberty procedures.
- Experience of multidisciplinary working.
- Knowledge and experience of carrying out Comprehensive Geriatric Assessment.
- Experience of medical education/clinical supervision.
