We are looking for a motivated, newly qualified individual who wishes to work with clients with diverse and complex mental health needs. Main duties of the job include:
- Assessing, planning, implementing, and evaluating evidence-based care, including health promotion for a designated service user or group.
- Conducting clinical risk assessments for service users contacting the Out of Hours service, providing advice, guidance, and decision-making, and coordinating care plans.
- Managing admissions, case conferences/reviews, and discharges in collaboration with the multidisciplinary team.
- Coordinating care and assessments under the Care Programme Approach for a caseload.
- Participating in service development, setting quality standards, and conducting audits in line with current clinical guidance, practice, and policy.
- Ensuring understanding and compliance with Safeguarding Vulnerable Adults and Children Legislation.
- Adhering to the requirements of the Mental Health Act 1983.
- Promoting carer and service user involvement within the service.
- Establishing therapeutic relationships and implementing evidence-based therapeutic interventions with professional boundaries.
- Ensuring carers\’ assessments, education, and support are delivered per NSF requirements.
- Providing support and coverage for colleagues and team members as needed.
- Completing mandatory training, participating in staff appraisals, and staying updated on trust policies.
- Providing management supervision where appropriate.
- Collaborating with key partners, service users, carers, clinicians, and practitioners within the multidisciplinary team.
