An exciting opportunity has arisen for a band 6 Nurse to develop skills in diabetes nursing. Supported by the Diabetes Specialist Nurse, Diabetes Nurse educators, Diabetes Nurse Associate and Diabetes Nurse Consultant, you will be working with the wider diabetes multidisciplinary team to provide specialist diabetes support, advice and education for people with type 1 and type 2 diabetes as well as other healthcare professionals.

We are seeking a nurse who has a positive approach and is committed to making a difference. You will need to demonstrate a keenness to work in the field of diabetes, effective communication and teamwork. Experience of supporting individuals with diabetes and carers with diabetes management would be an advantage but not essential as training and development in role can be provided.

The role will include working within a variety of clinical settings: the inpatients diabetes nurses assessing and reviewing people with diabetes who are in hospital as well as working within the outpatient department and some community based diabetes clinics.

The post holder will provide on-going specialist education and skills training for people with diabetes, carers and staff around management of diabetes as well as promote self-management strategies and provide opportunities for health promotion.

The post holder will contribute to the implementation of evidence based practice in line with national and local diabetes strategies and guidelines and will be given the opportunity to support service development or audit projects within the department.

The role is predominantly based in the diabetes centre with inpatient ward based clinical assessment and reviews and will also include some diabetes outpatient and community clinics. There is scope to develop knowledge in new advances in the care of those with Diabetes such as continuous blood glucose monitoring and insulin pumps and linking in with the wider Diabetes MDT such as medical team and Diabetes Specialist Nurses.

The post holder will carry responsibility for either a defined case load or offering specialist service advice, this will include setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care, management and education.

The post holder will contribute to achieving the objectives of the clinical service group/specialty corporate objectives and work within the clinical governance framework.

Working closely with the Diabetes Specialist Nurses to perform comprehensive assessment of nursing needs, plan, implement and evaluate care delivery according to changing health care needs. Providing individualised education to people with diabetes and carers to support diabetes management and promoting healthy lifestyle.

Supporting people with diabetes whilst they are in hospital within Aintree hospital, improving the patient safety, experience and outcomes and timely discharge back to place of residence. Collect, collate, evaluate and report information, maintaining accurate person centred records. Involve people with diabetes and carers/relatives in the planning and delivery of care.

Clinical sessions in a variety of settings including ward based reviews, outpatient clinics, and community clinics. Work collaboratively with other professionals and agencies to ensure individual person needs are met, especially in relation to ongoing care needs and discharge arrangements. Establish and maintain effective communication with people with diabetes and carers/relatives.

Managerial/Leadership

Provide specialist, expert clinical knowledge to clinical colleagues, people with diabetes and carers/relatives. Undertake clinical supervision of nursing colleagues on an individual or group basis. Participate in the development of services and assist the Nurse Consultant to implement change. Ensure the effective and efficient use of physical and financial resources. Monitor health, safety and security of self and others and promote best practice in the ward area.

Education and Development

Provide education and development opportunities for others including individual education and education of other health care professionals. Develop own expert clinical knowledge and participate in the development of others clinical knowledge. Ensure own compliance and compliance of others with regard to mandatory training and revalidation requirements.

Clinical Governance

Registered Practitioners have a responsibility to maintain current active professional registration, act always in accordance with their professional Code of Conduct and guiding documents, adhere to Trust Policy and Procedure, maintain up to date skills and knowledge and maintain awareness of professional issues, maintain a professional portfolio.
