We are seeking Occupational Therapists to support our innovative care coordination program. This role allows you to practice at the top of your license, expand into care coordination duties, standardize your hours, and advance your careerβall with flexible scheduling, excellent benefits, and the chance to impact seniors' lives positively.
Responsibilities:
- Overseeing care delivery in compliance with regulations
- Supporting care coordination through wellness visits, communication with providers and caregivers, and telehealth support
- Leading operational initiatives and implementing policies and programs
- Advising interdisciplinary care teams on treatment plans
- Maintaining professional and ethical standards
As an OT, you will be responsible for:
- Providing occupational therapy and wellness services to older adults
- Conducting assessments, planning treatments, and therapeutic interventions
- Contributing to program development and quality improvement
- Enhancing patients' functional well-being and satisfaction
Schedule & Benefits:
Our clinics typically operate 8 a.m. to 5 p.m., Monday through Friday, but flexible scheduling is available.
