Job Title: Band 6 Community Psychiatric Nurse (CPN)

What You\’ll Be Doing:

Join our team as a Band 6 CPN and play a vital role in supporting adults with mental health challenges. Your responsibilities will include:

1. Conducting comprehensive mental health assessments for adults in the community.
2. Developing and implementing personalized care plans in collaboration with a multidisciplinary team.
3. Providing evidence-based therapeutic interventions to support recovery and well-being.
4. Offering crisis intervention and risk management strategies to ensure patient safety.
5. Liaising with families, social workers, and other healthcare professionals to deliver holistic care.
6. Maintaining accurate and up-to-date patient records in line with NHS standards and guidelines.
7. Supporting service users with medication management and adherence.

Requirements & Qualifications:

To succeed in this role, you will need:

• A current and valid NMC registration as a Mental Health Nurse.
• Previous experience working as a CPN within an adult mental health setting.
• Strong communication and interpersonal skills.
• The ability to work independently and within a multidisciplinary team.
• A commitment to delivering high-quality, patient-centered care.
