Work as part of a team of Case Managers in the community care setting, providing the best possible outcomes for high-risk adults with long-term conditions at risk of hospital admission. Provide teaching and mentorship as the service develops.
Advocate for individuals with acute conditions requiring Case Management. Utilize advanced skills and expert knowledge to assess physical and social needs of clients with complex acute conditions, initiate therapeutic treatments including extended and supplementary prescribing based on evidence to improve health outcomes.
Anticipate short and long-term care needs, develop and adjust care plans, and refer to other services or professionals as appropriate. Recognize and assess the learning and development needs of colleagues; participate in creating, delivering, and evaluating training opportunities.
Communicate effectively at all levels within the organization, with health professionals, users, and carers to achieve optimal outcomes for older people.
Use advanced skills to assess subtle and unpredictable changes in patient condition. Conduct comprehensive and focused assessments using expert knowledge.
Collaborate with GPs, consultants, and Advanced Nurse Practitioners to diagnose, develop care plans, interpret complex situations, and follow up with patients with acute conditions. Negotiate roles and responsibilities with patients, carers, and care professionals, referring to other services as needed.
Challenge professional and organizational boundaries; identify and apply areas for skills and knowledge development to ensure high-quality, patient-centered care.
