The Old School Surgery and UWE Healthcentre is seeking a motivated Care Navigator to join their welcoming reception team. This role involves direct patient interaction, administrative duties, and scheduling appointments. Candidates should possess strong interpersonal skills, with hours up to 37.5 per week and flexibility for shifts, including occasional weekends.

Access to NHS pension, Cycle to Work scheme, Wellbeing team and Mental Health First Aiders, social events and themed activities, free fresh ground coffee and snacks.

  • Serve as the first point of contact for patients, answering queries and guiding them to appropriate services.
  • Perform administrative tasks, including processing documents and booking appointments.
  • Maintain professionalism when handling difficult situations.

This post is for up to 37.5 hours per week - Full and Part time considered. This will be made up of a mixture of early shifts (7.00am start), and late shifts (7.00pm finish). You MUST be available to work a blend of these shift times (we'll consider all work pattern requests). This will also include very occasional Saturdays. You will be required to work across both of our sites subject to business need.

Main duties of the job
  • Being a first point of contact for our patients, whether it be face to face or by the telephone, to offer support and guidance on choosing the right service at the right time.
  • Dealing with administrative tasks behind the scenes, processing incoming documents from third parties, prescriptions, GP requests and much more.
  • Being friendly and welcoming to all patients with the ability to remain calm and professional when dealing with difficult situations.
  • Using our online patient access system (Klinik) to monitor online patient requests, and triage patients accordingly.
  • Booking appointments for GP clinics and other practice clinicians as required ensuring correct information is provided.
  • Ensuring that accurate information and messages are passed to the Doctors and other surgery staff.
  • Dealing with all telephone queries/requests from patients and acting as a liaison with GPs and other practice clinicians.
  • To participate in in-house training, or other training as required.
  • To respect and maintain all aspects of confidentiality relating to Patients, Staff and Practice.
  • Any other duties which might reasonably be expected of the post.

Themed events such as: ice cream van in the car park, pancake day celebrations, cake baking, staff parties and once a Taco Van in the car park! Some of the wellbeing activities staff have participated in were: Mental Health Awareness Week Nature Competition + which team could grow the tallest sunflower where we gave a pack of seeds for everyone. We offer free fruit and biscuits every day. We are a large team and can therefore accommodate most working patterns. We love working here and we are sure you will too!
