Supported Assistant Practitioner/Assistant Practitioner 8241 | Permanent contract | £27,711 to £32,654 per annum (Scale F/G) | 37 hours per week | Norwich Community Hospital

We have a full-time opportunity in our Home First Hub, an integrated team working closely with Norfolk Community Care (NCH&C) and NHS colleagues to ensure safe and timely hospital discharges across the Central Area of Norfolk.

This is a new, developing team committed to supporting people to return home, access reablement opportunities, and plan their ongoing care needs after discharge.

As a Supported Assistant Practitioner (Scale F), you will join a 9-month bespoke support program, with the potential to progress to an Assistant Practitioner role (Scale G) upon successful completion.

You should possess experience in Health or Social Care, have a good education standard, and excellent IT and organizational skills. You must be capable of working in a fast-paced, responsive environment with a positive attitude. Your role will involve professional curiosity, independent work, or teamwork, with good listening and analytical skills. Flexibility and adaptability are essential, as is a commitment to supporting safe hospital discharges.

This is a hybrid role requiring hospital visits, face-to-face assessments, and attendance at the office at least three times a week, with remote work possible depending on connectivity. The role may involve assessing people in their homes or working within the Hub, aligned with the Discharge to Assess model.

Some weekend work will be required.
