We have an exciting opportunity in Telford Community Mental Service for a caring and compassionate Mental Health Nurse. We care for people who experience a wide range of mental health issues in Telford, offering assessments, treatment, and interventions. Our multi-disciplinary team approach is essential in formulating and providing high quality, safe and effective care, whilst ensuring our service users and families have a positive experience whilst under our care.

We are seeking a Community Mental Health Nurse to care for an identified group of service users, whilst also being able to gain experience in all functions of the team. We would like to appoint someone who has a real passion for working with service users in the community.

The community mental health service works as part of a 'neighbourhood' offering place based provision of mental health services in partnership with other local organisations. Clinicians within the community mental health service will offer interventions and treatments which are concordant with NICE guidelines, and allocation within the team will be based on the needs of the service user, matching clinician skills to service user needs.

You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the person to meet their agreed goals and coordinate and/or participate in the review of progress towards those goals, utilising recommended outcome measures.

Main duties of the job:

  • We offer a preceptorship programme to newly qualified staff; and would work with individuals coming into our team to ensure a personalised induction programme.
  • Allocated key worker to service users.
  • Care planning.
  • Risk assessment and management.
  • Delivering evidence based interventions.
  • Therapeutic 1:1 work.
  • Family work.
  • MDT working.
