Remuneration: £12.50 per UDA

Description: Part time Performer Dentist required for 2 days per week in a busy, mixed, well-established, INDEPENDENTLY OWNED practice near Hinckley, Leicestershire.

1. £12.50 per UDA, 2500 UDAs available
2. 50% split on Private & Labs
3. Tuesday to Friday
4. Fully computerised, Pearl Software, Digital X-rays
5. Existing, well maintained list
6. Full support team including a Hygienist
7. FD training practice
8. Parking on site
9. Available now, start date negotiable.

Compliance requirements:

• Full NHS Performer Number if applicable
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
• IRMER
• PIN number
