Occupational Therapist, Homecare

Under the supervision of the Team/Program Manager, the HH clinician is accountable for effectively managing or caring for a caseload of patients from admission to transition into the community, involving interdisciplinary support and referrals when appropriate. This position coordinates the interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement. These responsibilities will be carried out in accordance with organization policies/procedures and standards consistent with Federal, State, and accreditation regulations.

Requirements:

• Equivalent education required for RI licensure.
• Bachelor’s Degree in relevant discipline strongly preferred.
• A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or home health care setting preferred.
• Valid CPR certification required.
• Valid RI Board license.
• Valid driver’s license and automobile liability insurance are required.
