Our intermediate care services include:
- Rapid Response Services: In addition to our well-established Care Call services, we are one of seven nationally acclaimed accelerator sites for NHS England's Aging Well program, delivering a new urgent community response service for people experiencing health or social care crises. Our skilled team of health and social care professionals and carers work together to support people at home or in the community, helping to avoid hospital admissions or respite care.
- Intermediate Care at Home Services: Providing a wide range of care, social work, rehabilitation, therapy, equipment, and assistive technology to support individuals in living well and independently at home or in the community. We pride ourselves on our GOOD CQC registration and strong re-ablement outcomes, with over 80% of service users remaining independent at home three months after leaving our care.
- Hospital Discharge Team and Intermediate Care Bed Bases: Supporting the safe and timely transfer from hospital to appropriate settings, providing care and rehabilitation to regain independence and conducting assessments for long-term needs.
