The predicted salary is between 28000 - 42000 Β£ per year.
We are looking for an experienced agency social worker to join the Rhondda Cynon Taf East Learning Disability Team. The team works with adults with a learning disability in the East of the borough, covering roughly Pontypridd to Aberdare/Hirwaun area. The worker will be joining an experienced and supportive team.
To safeguard and promote the wellbeing of adults through provision of high-quality Social Work practice in assessment care planning and review. Undertaking assessments in line with the Social Services and Well-Being Act (Wales) 2014, identifying outcomes and where required developing care and support plans, which are monitored and reviewed. Undertaking Mental Capacity Assessments and working within the principles of the Mental Capacity Act 2005. Assessing risks in a balanced way that promotes coproduction, independence, and choice. Identifying and promoting individual strengths. Work in collaboration with other agencies and adopting a multi-disciplinary approach where appropriate. Ensuring that electronically held records, including case recordings, assessment, care and support plans and reviews are completed and/or updated in accordance with departmental policy and procedures. Requirement to travel independently throughout Service Area. The team work remotely with one office day a week.
