Community Learning Disability Nurse

This is an exciting developmental opportunity under Annex 20 for a band 5 nurse to work towards band 6 competencies. Achieving band 6 competencies will enable you to transition into a band 6 role.

It's a rare opportunity to join our friendly Community Learning Disability Team in North Tyneside. Our team is a large well established and supportive multi-disciplinary team offering holistic health care to children and adults who have a learning disability living in North Tyneside.

The ideal candidate will have an understanding of National and Local Policies relating to the management of people with learning disabilities.

Please note that previous applicants do not need to apply. Learning Disability student nurses who are due to qualify this year are welcome to apply for this post.

This post falls within the provisions of Agenda for Change Annex 20 - Development of Professional Roles, whereby upon completion of an agreed set of competencies you may move onto the next pay band without the need for an application for a post at a higher level.

The post holder will be welcomed as a member of the friendly community learning disabilities team in Northumbria Health Care Trust offering regular team meetings, robust support and supervision, and a commitment to continued professional development.

As an integral part of North Tyneside Community Learning Disability Team, the post holder will be solely responsible for a personal caseload including assessment, planning, implementing and evaluation of care. They will be passionate about working in collaboration with service users, their family, carers and professional colleagues in order to enhance health and wellbeing, to prevent or reduce behaviour that challenges.

We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate.

• To provide a high quality community nursing service to adults and children who have a learning disability within the North Tyneside area.
• To work across Primary and secondary healthcare services to ensure equality of access for those with learning disabilities.
• To work with other agencies and services (statutory and non-statutory) to ensure equality of access for those with a learning disability and facilitate effective communication between clients and other agencies.
• To provide community nursing support to adults and children who have behavioural/mental health problems. Support the design and implement Positive Behaviour Support Plans to provide intensive, proactive support and crisis intervention to patients, carers and care providers to prevent/reduce admission to in-patient units and to maintain community placements.

• Essential criteria: Registered Learning Disability Nurse, Mentorship Qualification or relevant equivalent qualification (essential requirement for post, expectation this will be completed within an 18-month period), R.N.M.H/R.N.L.D. Qualification.
• Desirable criteria: Registered Mentor.

• Essential criteria: Evidence of ongoing professional development, experience working with people who present with challenging behaviours and experience of working within a PBS framework, experience of multi-agency working, experience working with children and adults, experience working in the community setting, evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period).
• Desirable criteria: Knowledge of or experience in coaching and mentoring practices and tools, knowledge of or experience in Quality improvement tools, techniques and methods.

• Essential criteria: It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role.
