We are looking to recruit a Reablement Co-ordinator on a permanent basis on a -hour contract who truly care and are committed to making a difference in an individual’s life.

Being a Reablement Coordinator means supporting people in their own homes for a short period to assist them in reaching their full potential. This may include managing various aspects of daily living skills, setting clear and achievable goals to help people rebuild their independence and confidence to sustain independent living at home and in their communities.

You will work effectively within the reablement team to deliver high-quality reablement services aimed at promoting and sustaining people’s independence in the community. Supervising staff and mentoring the personal development of Reablement workers are also key responsibilities.

A driving licence is essential for this community-based role.

Ensure the successful completion of service users’ journeys, coordinating and complying with organisational policies, procedures, and Care Quality Commission regulated guidance. Strive to achieve the best quality outcomes for service users, their carers, and families.
