Location: Hampshire (Winchester and Basingstoke area)

Salary: £22,932 (£18,345 for 28 hour week) per annum, pro rata

Hours: 28 per week (typically working 10am - 6pm, Mon, Tue, Thur, Fri each week)

Contract: Fixed term contract (30th September 2025, contract likely to be extended)

Driving Requirement: Full UK licence, at least 2 years

Our service support workers are a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. You'll allow people to enjoy healthier, more fulfilling lives. A day in the life of a Service Support Worker will involve:

  • Providing practical and emotional support (telephone calls, welfare checks, shopping etc) and guidance to service users.
  • Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
  • Responding to referrals from Clinicians and Community Health Professionals.
  • Assessing service users' needs, completing and following a support plan for the individual.
  • Supporting service users in the weeks after hospital discharge or to prevent readmission.
  • Taking someone to a hospital appointment that they otherwise couldn't get to or picking up prescriptions and shopping.

To be a successful Service Support Worker, you'll need:

  • A good knowledge of services provided by the NHS and Social Care.
  • The ability to improve service quality for the benefit of users.
  • Professionalism to deal with queries in a diplomatic and confidential manner.
  • Flexibility in working hours.

There is no personal care or administration of medication for this role.
