Job Summary:

Performs the functions of a Licensed Practical Nurse in assessing, planning, implementing, and evaluating assigned clients’ care in collaboration with the RN Nurse Coordinator. Requires a high degree of independent judgment.

Qualifications:

Graduate of an accredited school of nursing as a Licensed Practical Nurse (LPN)

Unencumbered Nursing license.

Uniform Assessment Instrument (UAI) and Individual Service Plan (ISP) trained or obtain within six months of employment

Minimum three (3) years supervisory experience as a nurse
