MSW - Social Worker

We Are Hiring

• Department: Family Practice
• Schedule: Monday-Friday 8am-5pm
• Clinic: Ascension Medical Group
• Location: 1121 S Clifton Wichita, KS. 67218

What You Will Do

Investigate, assess, and plan interventions to help patients cope with social, emotional, economic, and environmental problems.

• Interview patients, assess priorities, and document case activity.
• Consult with team managers, statutory and voluntary agencies, and patient\’s relatives to ensure compliance with the treatment plan.
• Assist clients and families with obtaining community assistance by referral to proper resources.
• Develop and maintain working relationships with community health, welfare, and social agencies, seeking creative means to assist clients with their needs.
• Support management of concurrent review, discharge planning processes, social services, home care, and other post-discharge needs.

What You Will Need

Licensure / Certification / Registration:

• Social Worker credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.

Education:

• Master\’s degree in Social Work required.

Work Experience:

• 3 years of experience preferred.
