Responsibilities:

1. Review and refine care and support plans, ensuring compliance with statutory timescales such as those under the Care Act 2014 or Children Act 1989.
2. Conduct assessments of changing needs and update care packages accordingly.
3. Ensure service delivery is cost-effective, person-centered, and adheres to safeguarding policies.
4. Engage with service users, families, providers, and multidisciplinary teams to foster collaboration.
5. Maintain accurate case records and complete reports promptly.
6. Promote independence and advocate for necessary services or modifications in care.
