About the Role:
Liverpool City Council is seeking a dedicated and compassionate Outreach Family Support Worker to join the Liverpool Family Help team. This rewarding role focuses on delivering early, strength-based interventions to children and families, aiming to address emerging concerns and improve long-term outcomes.
Key Responsibilities:
- Conduct Family Help Assessments with families.
- Develop and implement tailored support plans.
- Lead and coordinate Team Around the Family (TAF) meetings to monitor progress and identify evolving support needs.
- Work directly with children and young people to understand their views and ensure their voices shape support planning.
- Collaborate with a wide range of multi-agency partners to deliver a whole-family approach to care and intervention.
