An exciting opportunity has arisen for a highly motivated and proactive Discharge Navigator to join the Transfer of Care Team at the Royal London Hospital. This joint health and social care team plays a vital role in facilitating safe and timely discharges based on the Home First model, ensuring patients return to the most appropriate community setting as soon as they are medically fit.

Key Responsibilities:

  • Act as the discharge planning expert on the wards, supporting patients who no longer require acute care
  • Coordinate with ward MDTs, senior management, and community services to support safe discharge planning
  • Provide up-to-date advice and guidance on community resources, including local authorities, voluntary services, and post-discharge support pathways
  • Work collaboratively to reduce delays and ensure smooth transitions of care
  • Advocate on behalf of patients and problem-solve barriers to discharge
  • Maintain a strong understanding of government discharge guidance and local protocols
  • Supervise junior staff and support their development within the discharge planning process
  • Operate across clinical and non-clinical areas, supporting patient flow and system efficiency
  • Manage and prioritise a high-pressure workload in a fast-paced acute hospital environment

Requirements:

  • Previous experience in discharge planning or care coordination in an acute or community healthcare setting
  • Sound understanding of discharge processes, social care pathways, and multi-agency collaboration
  • Ability to work confidently with senior stakeholders and external agencies
  • Excellent organisational and communication skills, with the ability to manage competing demands
  • Strong team player with the confidence to challenge decisions and escalate concerns when necessary
  • Flexibility to adapt to a dynamic and evolving hospital environment

Desirable:

  • Clinical background in Nursing, Physiotherapy, or Occupational Therapy is advantageous but not essential
  • Experience supporting or supervising junior staff is preferred
