As a Social Worker within the Safeguarding Front Door (Contact) Team, you will be the first point of professional contact for adults at risk of abuse or neglect. You will play a pivotal role in screening, triaging, and risk-assessing safeguarding concerns in line with the Care Act 2014 and Southwark's Multi-Agency Safeguarding Procedures. You will work collaboratively across agencies, making informed decisions based on presenting risks, mental capacity, and available evidence to determine whether a Section 42 Safeguarding Enquiry is required. You will ensure timely, person-centred, and legally compliant responses to safeguarding referrals.
Key Responsibilities:
- Receive, screen and prioritise incoming safeguarding referrals via the Contact Team.
- Conduct initial risk assessments to determine threshold for Section 42 enquiries.
- Liaise with referring professionals, families, health, police, and internal services to gather further information.
- Apply the Making Safeguarding Personal (MSP) approach at the earliest opportunity.
- Clearly document decision-making and outcomes, escalating high-risk cases as appropriate.
- Provide professional advice and guidance to other staff, partners, and the public on safeguarding thresholds and Care Act criteria.
- Identify immediate safeguarding actions or urgent interventions, including safety planning or emergency measures.
- Support safe and appropriate signposting or onward referrals, including to specialist teams (e.g. Learning Disabilities, Mental Health, Hospital Discharge).
- Maintain accurate and timely case records on Mosaic in accordance with statutory and organisational standards.
