We are seeking an experienced and dedicated Band 6 Community Nurse to join our Substance Misuse Team, specialising in drugs and alcohol. This is a full-time role working Monday to Friday, offering a competitive hourly rate of £25.75 (umbrella). Key Responsibilities: Provide expert nursing care to individuals affected by drug and alcohol misuse within the community. Conduct comprehensive assessments, including physical and mental health reviews, for clients with complex needs. Develop and deliver tailored care plans, in collaboration with multidisciplinary teams, that focus on harm reduction, detoxification, and long-term recovery. Administer medications, including substitute prescribing (e.g., methadone), and monitor withdrawal symptoms. Offer advice and support on safer drug use, health promotion, and overdose prevention. Liaise with GPs, social services, housing, and other external partners to provide a holistic approach to client care.
