As a lead professional for people with mental health needs or learning disabilities you will work as part of the continuing health team as a case manager to ensure that individual service users receive a high quality of care and support in your commissioning role. You will work alongside other professionals within the multidisciplinary team and here in Calderdale we have very positive working relationships with other statutory and provider agencies in assessing and meeting individual needs., Working within our team will enable you to have the opportunity to work on and develop key areas of interest to enable you to develop your professional portfolio. There are opportunities for training and also career development. The role may be particularly attractive to professionals from mental health nursing, social work or occupational therapy backgrounds.

You will be working with a wide range of service users whos primary health needs will be based on Mental Health, Learning Disability or ASD. Experience in working with people with complex mental health needs will be essential to this role as you will be instrumental in care planning to help people fulfil their potential. This role is varied, and no two days are ever the same, you will have lots of exciting opportunities to develop your skills. You must possess excellent communication & intrapersonal skills, and a good working knowledge of complex mental health and related legal frameworks., Have subject matter expertise and programme management skills to lead system change across Learning disabilities and mental health issues within Calderdale & West Yorkshire;

Have a good working knowledge of Mental health act, Mental capacity act, S117 and other legal processes.

Be able to work within the National Framework.

be resilient, flexible and adaptable, working to tight deadlines as part of local, regional and national systems;

build & maintain strong relationships, working collaboratively with family carers, young people, older people & system partners in Calderdale and West Yorkshire.

facilitate the delivery of measurable, sustainable, value-for-money improvements that improve people with learning disabilities / mental health problems outcomes, impact, and experiences.

Organise and chair Care and Treatment Reviews and Care and Educational Treatment Reviews alongside a panel of experts. The ability to challenge poor practice.
