Maintain good performance within the team completion of care plans, Care Act Assessments, care reviews and Mental capacity assessment and Best Interest Meeting To be fully responsible for managing a caseload including people with complex needs in adherence with Care Act and Mental Capacity Act Work in the office twice a week

Main Responsibilities of the Job

  • To ensure compliance with the professional code of conduct and fulfil obligations

under the relevant legislation, including the Care Act 2014, CHC, the MCA and the

MHA.

  • To be fully responsible for managing a caseload including people with complex

needs, (including mental health).

  • To be responsible for accepting referrals, undertaking screening, and carrying out

assessment of needs for people.

  • To apply eligibility criteria as identified in the Care Act and work in accordance with its

requirements to ensure people’s well being.

  • To assist vulnerable adults to be protected and safeguarded and ensure that they are

effectively managing their own risks and capacity wherever possible.

  • Ensure that a strengths based, person centred approach is central to your

interventions

  • To discharge statutory duties related to the care of adults and of their carers.

  • To maintain the professional qualifications, memberships and registrations required

to perform the role, including keeping updated CBD.

  • To develop and maintain effective working relationships with all partners (both

internal and external).

  • To promote and participate in good team working practices and to demonstrate these

whenever possible.

  • To ensure both staff and public safety, by preventing and diffusing difficult situations

and taking responsibility to escalate to managers where necessary.

  • To keep up-to-date with developments, products and information relating to the

service and the rest of the council to ensure that appropriate and timely information is given to people.

  • To ensure that proportionate risk assessments for both the person and the carer

associated with any support plan have been carried out.

  • To undertake reviews of people with long term needs as required and within statutory timelines and carry out reassessments as required.

  • Undertake BIA training where appropriate.

Educated to degree level with appropriate professional qualification relevant to the role and specialism (e.g.- CSS/CQSW or DipSw Dip COT or Degree in Occupational Therapy)

  • Registered with SWE

  • Knowledge of the Care Act 2014,,the Mental

Capacity Act 2005 and other relevant legislation.

  • Demonstrable understanding of the principles and application of excellent adult social care

  • Demonstrable experience of creating effective working relationships with internal and external customers

  • Strong literacy, numeracy, interpersonal and IT skills.

  • Relevant experience of working in a Social Care setting or other relevant agency
