The predicted salary is between 36000 - 60000 £ per year.

You will be assessing patients in this job on your own to determine their acuity and their progress in following their treatment plan or recovery goals.

Provide clinically sound interventions such as catheterisation, physical assessments, tissue viability assessment and care, phlebotomy, ECG recording and basic interpretation and physical assessments.

Exhibit safe clinical decision-making, including evaluation and diagnostic abilities.

You will be interpreting test results and making clinical decision autonomously using your evidence based clinical knowledge.

Each day you will benefit from and actively participate in a Geriatrician led MDT that includes PTs, OTs, Nurses, associate actioners and pharmacists.

You will be expected to see patients in their own home independently with support from the MDT and wider ECP services.

As a centre of excellence, you will deliver a person-centred approach to their independence and wellbeing.

Part of this role will include triaging patients who are referred from different stakeholders such as GPs, London ambulance Services and acute hospitals.

Being part of ECP you will work in A&E to identify patient appropriate for rapid response and prevent in hospital admission avoidance. You will also work closely with DNs, TVN, community therapy, End of Life care amongst others allowing you to make meaningful treatment plans for our patients.

Independent home visits or joint visit if needed.

Undertake physical assessments of patient, diagnose, plan, carry out, and review the care given to patients who have undifferentiated diagnoses.

Making decisions independently based on your evaluation, diagnosis, and interpretation of test results.

Documentation

Provide support to wider MDT

Act as first point of contact (SPA) for the Rapid Response and Admission Avoidance Service.

Discharge patient safely from the caseload by completing the discharge summaries and informing the patient.

You will have opportunities to provide supervision and line management to agreed cohort Rehabilitation Support Workers.

Ensuring safer staffing on a day to day basis in absence of band 7s.
