We are looking to recruit a NMC registered nurse to work within the Rapid Response team in Amber Valley and Erewash on a 12 month fixed term contract.

Our urgent (Rapid), community response team assess and provide individualised patient care, prioritising need to ensure our patients get the most efficient and clinically effective treatment in their own homes. This post holder will be skilled in promoting self care to our patients and have a drive for excellence as they join our community nursing services.
Evidence based high quality care is our aim in Rapid Response as we actively support the wider community nursing and therapy teams visiting patients who require a new or urgent episode of care. This post holder will work in close collaboration with the wider Integrated Community Teams/GP surgeries and local acute trusts.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
