As part of their continued growth, they are seeking an experienced and qualified Independent Prescriber Pharmacist to join our multidisciplinary team. Key Responsibilities:
- Conduct independent clinical assessments and issue prescriptions within your clinical competence.
- Work collaboratively with GPs, nurses, and other healthcare professionals to support patient care pathways.
- Lead on medicines optimisation initiatives, including medication reviews and audits.
- Support the management of chronic conditions such as diabetes, hypertension, asthma, and COPD.
