Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team. The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.
Responsibilities include:
- Work within the discharge to assess model working with customers within the hospital and the pathways.
- Collaborate with the Home First and Occupational therapy team and closely with Carelink & Telecare, LAC, Locality teams, and Brokerage.
- Complete assessments in all 3 pathways with the aim of returning customers to their homes.
- Be responsive and able to work on your own initiative, have experience of assessing and reviewing customers, with knowledge of equipment and a re-ablement ethos.
- Attend Multi Disciplinary meetings and possess excellent communication, IT, and organizational skills.
- Travel across the City of Derby; must have a full UK driving licence, use of a car for work purposes, and business use insurance or alternative transport.
- Liaise with other professionals.
- Work 36 hours per week (flexible by negotiation).
2 Month Temporary Contract. Start date subject to DBS and reference clearance.
