As part of the expansion of our Community services development, we are looking to recruit Band 3 Health Care Assistants with a Rehab role to support our Community hub in Guildford and Waverley. We are passionate about supporting patients at home, preventing unnecessary hospital admissions and supporting people when they are discharged from hospital. You will become part of our team working with Occupational Therapists, Physiotherapists, and Nurses to provide short-term care and rehabilitation to patients in their own homes to help them regain their independence. You will deliver care to patients whilst also supporting their rehabilitation programme as overseen by the registered staff.

Main duties of the job:

  • You will be supported with training and career development opportunities which include a Nationally recognised care certificate and local team competency programme.
  • You will have the opportunity to develop your expertise in a community setting with patients with a range of conditions.

We operate 7 days per week from 8 am to 9 pm.

Detailed job description and main responsibilities:

  • Patients will require a range of short-term home support to keep them at home or to help them once they are discharged from hospital to achieve their optimum independence.
  • Individuals will require support with personal care, mobility, medication administration, and exercise programmes to achieve their individual recovery and rehabilitation goals.
  • To deliver care in accordance with National and local priorities, Trust Policies, procedures, and guidelines.

Person specification:

  • NVQ Level 3 or equivalent level of experience
  • Interest in working in a community setting (patients' own homes)
  • Previous experience of delivering patient care
  • Experience of working in a community setting
