The ACT Team Nurse (RN) will provide in-home services to clients with severe and persistent mental illness. Position is responsible for meeting the nursing service needs of agency consumers by providing competent standards of care in direct and indirect care roles. The RN will collaborate with other ACT members to complete comprehensive client assessment through effective interviewing, behavioral observations and mental health assessments in order to reach sound conclusions and plan appropriate interventions with the consumer. The RN will combine biopsychosocial knowledge and psychopharmacological competency to identify actual and potential health problems. The RN will prioritize needs and treatment approaches to ensure appropriate delivery of care. The RN will coordinate care with physicians, nurse practitioners, and other agency and external service providers as required. This position involves shift coverage for ACT Team, including morning and evening hours, as well as rotating on-call duty overnights and holidays.

QUALIFICATIONS:

  • A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
  • Graduation from an accredited nursing program and licensed as a Registered Nurse or Licensed Practical Nurse in Virginia required.
  • Experience in psychiatric nursing or recent EHR experience is preferred.
  • Current Basic Cardiac Life Support Certificate or eligibility required.
  • Must possess a valid Driver's License, good driving record, and the ability to provide own transportation.

ADDITIONAL REQUIREMENTS:

  • May be exposed to allergenic environments from clients including dust, smoke, pet contamination, etc.
  • May be exposed to clients with infectious diseases.
  • Must be able to drive for long periods of time to visit other clinic sites in catchment area as needed.
  • Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles.
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to stand for periods of time.
  • Must be able to lift/push/pull up to 25 pounds at times.

SALARY: Starting at $52,788.00
