An exciting opportunity has arisen for a band 7 Nurse to join our East Riding Diabetes Specialist Team. The post holder will be welcomed into a small and supportive diabetes team. We are looking for a highly motivated and passionate band 7 nurse who can provide clinical expertise in the delivery of diabetic care. This is a part-time position working a few days a week between Mon-Fri.
Main duties of the job:
- To be accountable in own area of clinical responsibility, ensuring the delivery of evidence-based clinical interventions and practice for a specialist caseload in diabetes care.
- Working as part of an integrated team for adults diagnosed with diabetes.
- To continue to demonstrate specialist clinical skills in practice and provide support, training and advice regarding the delivery of care in partnership with patients, relatives, staff, managers, and externally with other agencies.
- To be responsible in conjunction with the team lead for organising the caseload and for ensuring referrals are triaged and stratified.
- Accountable for the high quality services to adults with complex needs arising from a diagnosis of diabetes.
- To ensure that patients receive the appropriate level of assessment and intervention.
- To support clinical lead in service development and monitoring service to ensure a “Fit for purpose” service.
