CHC Nurse Practitioner Required in Cardiff for 3-Month Contract

Excellent hourly rates.

We are working with an esteemed client based in Cardiff and urgently require a Nurse Clinical Practitioner for case reviews of patients with Continuing Healthcare needs (CHC). This will be a 4-month project covering 120 case reviews and is full-time Monday – Friday. The role will be site-based predominantly but will include some travel to patients\’ homes.

Responsibilities:

1. Initial reviews
2. Arranging and facilitating DST reviews and recommendations
3. All appropriate recording and documenting outcomes

Minimum Requirements:

1. NMC Registered Nurse
2. Current experience working in the same or very similar role
