SW Role:

They will assess social determinants of health by completing biopsychosocial assessments with the goal of increasing access to quality health care and reducing barriers to discharge. They will provide patients and families with emotional and resource support by providing interventions that are strength-based, focused on the whole-person, trauma-informed, and span across the life cycle of our patients. They will collaborate with the multi-disciplinary teams on how the illness or injury may impact the patient’s, family, employment, and living situation. They will promote health and reduce health inequities by advocating for, educating, and empowering patients and families.
