About the role

We are looking to recruit a Relief Days & Nights Relief Care and Support worker level 2 to join our Team on a permanent basis.

Care and support duties

  • Carry out duties as identified in care and support plans, including working with service users who have complex or high needs.
  • Implement specific programs of direct & virtual work with individuals on a 1-1 or family/group basis, collaborating with other professionals and agencies.
  • Ensure adherence to safe working systems daily, meeting requirements under RIDDOR, COSHH, manual handling, first aid, and medication.
  • Respond to emergency situations appropriately (e.g., contacting a doctor, ambulance service, fire service, police service).
  • Take responsibility for the safe handling of property and equipment belonging to the service user.
  • Administer prescribed medication, including controlled drugs, to service users.
  • Maintain good communication and develop effective working relationships with service users.
  • Identify and manage short- and medium-term risks to ensure service user safety, escalating risks to appropriate professionals.
  • Deliver preventions and interventions, including complex cases, to support individuals\’ mental, social, and physical wellbeing.
  • Work in partnership with individuals, their families, and other agencies using a strengths-based approach.
  • Undertake personal care as directed by the care and support plan, following correct manual handling techniques.
  • Utilize Positive Behaviour Management or Team Teach techniques as directed by the care and support plan.
  • Provide witness statements for legal proceedings when necessary.
  • Maintain up-to-date recording systems as required to document your work.
  • Work a rolling rota of days or nights, with expectations to cover shifts as needed.
  • Undertake any other duties commensurate with the level and expectation of the post.

Quality Assurance

  • Report all complaints and compliments to the Management Team.
  • Advocate for and with service users, families, and others.
  • Facilitate and participate in regular service user meetings.
  • Maintain appropriate records in accordance with Council or Service requirements.

General Responsibilities

  • Actively promote the Welsh language and service user language preferences.
  • Promote equality of opportunity in service delivery.
  • Work as a team and recognize your role and responsibilities within it.
  • Maintain confidentiality of information regarding service users, visitors, and staff.
  • Complete all required H&S and supervision records in accordance with relevant policies.
  • Promote and respect service users\’ rights to privacy, dignity, and choice.
  • Accompany service users in maintaining community connections.
  • Develop effective partnership working with internal and external colleagues.
  • Adhere to relevant statutory legislation and guidance, internal policies, and procedures.
  • Have knowledge of relevant National and Local Strategies.
  • Attend team meetings, supervision sessions, and training courses as directed by the Line Manager.

  • Extended Support: Adult and Children’s Disability Team & Mental Capacity Specialist Support Team
  • Substance Misuse: Substance Misuse Team, Youth Justice Statutory Team
  • Mental Wellbeing: Mental Health and AMHP Team, School Counselling Team
  • Safeguarding & Quality Improvement: Adult and Children’s Safeguarding Team
