What will your responsibilities be?
In this role, you will support adults and young people with learning disabilities by completing Care Act Assessments and reviews.
You will work collaboratively within a multi-disciplinary team, including Adult Social Care and NHS Health staff.
Managing safeguarding cases, assessing risk and coordinating care management effectively are key responsibilities.
You will engage with service users, families and partner agencies to promote person-centred support.
Demonstrating strong communication, organisational skills and a commitment to improving outcomes for individuals with learning disabilities is essential.
