Bristol City Council is seeking an experienced Social Worker to join our Adult Social Care Review Team. This is a fantastic opportunity to contribute to person-centred care planning and ensure services continue to meet the needs of adults across the city.

Working in a hybrid capacity, you’ll conduct strength-based reviews, working both remotely and face-to-face with service users, carers, and multi-disciplinary professionals.

Key Responsibilities:
  • Complete Care Act-compliant reviews of care and support plans
  • Promote independence and identify opportunities for care reduction where appropriate
  • Work in partnership with health professionals, providers, families, and advocates
  • Record assessments and updates in line with council policies and legislative requirements
  • Prioritise workloads effectively to manage a varied caseload
Requirements:
  • Qualified Social Worker with current Social Work England registration
  • Proven experience in adult social care, ideally within a review or assessment team
  • Sound knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding frameworks
  • Strong written and verbal communication skills
